When reporting an incident or a near-miss, it's essential that your EHS forms allow you to capture the correct data. Someone reporting an injury shouldn't have to fill in fields irrelevant to their case. Pro-Sapien forms have been designed the user in mind. The dynamic forms update as they're filled in, meaning that the form will ask you only relevant details based on your selections.
For example, if you report an injury, the form will update to facilitate this, whereas if you report another type of safety issue such as faulty equipment, the form will change the fields accordingly. On top of this, you will also get to specify the people involved, and employees can be searched from a look-up menu.
But how does this work in practice? In this short video, one of Pro-Sapien developers Reza Hosseinzadeh takes you through an incident report form to demonstrate their dynamism in action.
